Fill-out the Referral Form at the bottom of the page. Indicate psychological assessment and your concern. A member of our team will contact you to schedule an appointment. Once an appointment is scheduled, an email will be sent to you containing a child history form and any additional questionnaires or rating scales as deemed appropriate to assess your child’s needs. We request that all forms be completed prior to the first testing session.
